Call (718) 892-5520 to access the R.A.I.N. Case Management Program, which serves home bound seniors 60 years of age and over residing in Bronx Community Districts 7,8,11, and 12. Case Managers can assess and link seniors to a wide range of services such as transportation, housekeeping, home care, home delivered meals and assistance with entitlements and benefits.
